Content
Welcome to the September release of Merative Integrated Care. Care teams and administrators can now add progress notes to services for a client and add multiple notes to client barriers. Administrators can configure Barrier and Service notes types to enable editing of notes, and they can configure service notes and comments to be included on a Share Care Plan template. There are reporting updates available in the following areas: goal progress values, goal outcome values, action outcome values, action progress values, action progress values, and action values.

So that you can more easily identify the changes that will apply to your instance of Merative Integrated Care, updates are tagged as follows:
- Merative Integrated Care - these updates are for all Merative Integrated Care customers.
- Reporting - these items apply to all Merative Integrated Care customers using Merative Integrated Care Reporting.
- Service Providers - these items apply to Merative Integrated Care customers with the Providers feature enabled.
- Organization Units - these updates are for all Merative Integrated Care customers with Organization Units enabled.
- Community Service Referral - these items apply to customers who have licensed the Merative Integrated Care Community Service Referral add-on.
- Community Service Payment - these items apply to customers who have licensed the Merative Integrated Care Community Service Payment add-on.
- Connect Providers - these items apply to customers who have licensed the Merative Integrated Care Connect Providers add-on.
- Connect Individuals - these items apply to customers who have licensed the Merative Integrated Care Connect Individual add-on.
- Connect 360 - these items apply to customers who are integrated with the IBM Health and Human Services Connect 360 application.
Service notes
Merative Integrated Care
So that you can record progress on a client's service, you can add notes for services from the Plan Workspace, Services list and Service home page. If configured to be editable, you can save a service note that you want to return to later as a draft or ready for review version, or if you are finished editing you can save the note as the final version. You can comment on service notes as needed, and delete notes that are no longer relevant. The associated service notes is displayed on the client's Notes list with the note type set as 'Service'.
Barrier notes updates
Merative Integrated Care
Previously, only one note could be added to a barrier on a client's care plan. So that you can more easily record updates on a client's barrier, you can add multiple notes for client barriers. If configured to be editable, you can save a barrier note that you want to return to later as a draft or ready for review version, or as the final version. You can comment on barrier notes as needed. The associated barrier notes is displayed on the client's Notes list with the note type set as 'Barrier'. You can delete barrier notes from the client's Notes list.

Merative Integrated Care
So that you can provide a complete update on services when sharing the care plan, service notes and comments are included in the Share Care Plan PDF, if configured by your administrator for the selected template.

So that users of the care team application can record progress notes for services and barriers on a client's plan workspace, the library of Note Types is now updated to include Barrier and Service Note Types. You can configure note sharing options, associate an external system reference and enable editing of Barrier and Service note types by the care team members. If you have already configured note types for Barriers or Services, you should relabel these note types to avoid duplicate values. Users of the care team application can continue to add notes with the existing configured note types from the Notes list. However, users can only add notes with the new Barrier and Service note types from the plan workspace, Service list or home page or Barrier list or home page.

Merative Integrated Care
So that care team members can include service notes and comments when generating a Share Care Plan PDF, you can configure service notes and comments as part of a Share Care Plan template.
Goal progress values

Previously the goal progress values that were displayed in the goals reporting tables were the out of the box progress values, in 21:07 we made the goal progress values configurable in Merative Integrated Care. If progress values have been configured by a Merative Integrated Care administrator, these values are displayed when reports are run that contain the goals progress data item.

Previously the goal outcome values that were displayed in the goals reporting tables were the out of the box values, in 21:07 we made the goal outcome values configurable in Merative Integrated Care. If outcome values have been configured by a Merative Integrated Care administrator, these values are displayed when reports are run that contain the goals outcome data item.
Action outcome values

If outcome values have been configured by a Merative Integrated Care administrator, these values are displayed when reports are run that contain the care team and client action outcome data items.

If progress values have been configured by a Merative Integrated Care administrator, these values are displayed when reports are run that contain the actions progress data item.
Action category values

Previously the action category values that were displayed in the care team and client action reporting tables were the out of the box action values, in 21:07 we made the action category value configurable in Merative Integrated Care. If category values have been configured by a Merative Integrated Care administrator, these values are displayed when reports are run that contain the care team and client action category data items.
Merative Integrated Care
- When you view Progress values in the Tenant Admin application, the Display value for the 'None' progress value is incorrectly displayed as Hidden. This does not impact the care team application and users of the care team application will still be able to record 'None' as a progress value for goals and outcomes. (272704)
Merative Integrated Care
- If you try to complete certain actions, for example, create a referral or assign a program, and you do not have access to the client, you cannot complete the action and no error message is displayed. If you are sure that you need access to the client, either request emergency access to the client or ask a care team member to add you to the client's care team. (259832)
- If you edit a date data attribute for a client data type, and select Current Date Comparison, then Custom Message, and enter an incorrect message ID, the custom message fields are no longer visible. To see the fields, select None, then re-select Current Date Comparison and Custom Message. (264393)
- When you configure a client data attribute to not use the default value, the value is still visible to care team members. As a workaround, care team members can delete the default value and enter another value. (263255)
- When you configure an information message for a client data type cluster, the message is displayed twice. To avoid this, you can add a description to the data cluster instead of an information message. The description displays to care team members below the cluster title. (263229)
- If you delete a data attribute from a client data type and that attribute was used in a duplicate validation, the validation is not automatically deleted. If you encounter this issue, you can manually delete the validation. (264986).
- When you configure the Pending goal outcome with a display value of Shown, goal completion details are not displayed when care team members select Pending as the outcome. To avoid this, do not set the Pending goal outcome to Shown. (272034)
- When you add progress to a goal in the care team application, only the initial progress value is displayed on the Goal list page, and subsequent progress updates are not displayed. To see the latest progress value for the goal, refer to the Plan Workspace or the Goal home page. (272827)
- When multiple resource groups with the same category, gender and client type are configured by a provider in Connect P, only the resource groups with available resources are displayed to users of the care team application on the Community Resources page, and any resource groups with unavailable resources only are not displayed. - The Share Care Plan PDF does not display notes that are received from the configured external systems when care team members download or send the PDF. (272981)
- Deleted Service Notes are displayed in the Services section of the Share Care Plan PDF, if your administrator has configured Service Notes for the selected template. (273315)
- Barrier and Service Note Types are incorrectly displayed when you configure the Notes section of the Share Care Plan template in the Tenant Administrator application. This does not impact the care team application as the generated Share Care Plan PDF does not display Service and Barrier Notes in the Notes section. The barrier and service notes are correctly displayed in the Barriers and Services sections of the Share Care Plan, if configured by the administrator for the selected Share Care Plan template. - When the Client Name Display Format configuration is set to FirstName, LastName in the Tenant Admin workspace and, in the care manager workspace, you search for a client using their identification (for example, password number or social security number) or date of birth, the client's name is not returned in the search results. If you expand the search results to view them in more detail, the client's name is listed after the row label "Other Names". (273006)
- You can deactivate a client from your Unassigned Clients list. If you do and choose to close their client account, you encounter an error. You can proceed to deactivate the client without closing their account. However, if you do, the client will appear as deactivated in Integrated Care but they still incorrectly retain access to their Connect Individuals account. On deactivation, a client shouldn't be able to access their Connect Individuals account. (274018)
Reporting
- When SSO is enabled, if you sign out of the reporting application, you are automatically signed back in. If you experience this issue, sign out of the Merative Integrated Care application, then sign out of the reporting application. (263876)
Connect 360
- When you search for a client in both systems a misleading error message is intermittently displayed. The message indicates that the search results show only clients. The message displays only when the client you searched for was originally registered in and subsequently registered in Connect 360 automatically. Ignore this message, the client exists in both systems and is shown in the search results as expected. (256209, 252716)
