Content
Welcome to the March release of Merative Integrated Care. Care teams can now see a new client summary card and new touchpoint filters. They can also assign or reassign clients to other care team members. Administrators can now configure alerts to autogenerate from assessments and questionnaires. In addition, there is new data in Reporting.

So that you can more easily identify the changes that will apply to your instance of Merative Integrated Care, updates are tagged as follows:
- Merative Integrated Care - these updates are for all Merative Integrated Care customers.
- Reporting - these items apply to all Merative Integrated Care customers using Merative Integrated Care Reporting.
- Service Providers - these items apply to Merative Integrated Care customers with the Providers feature enabled.
- Organization Units - these updates are for all Merative Integrated Care customers with Organization Units enabled.
- Community Service Referral - these items apply to customers who have licensed the Merative Integrated Care Community Service Referral add-on.
- Community Service Payment - these items apply to customers who have licensed the Merative Integrated Care Community Service Payment add-on.
- Connect Providers - these items apply to customers who have licensed the Merative Integrated Care Connect Providers add-on.
- Connect Individuals - these items apply to customers who have licensed the Merative Integrated Care Connect Individual add-on.
- Connect 360 - these items apply to customers who are integrated with the IBM Health and Human Services Connect 360 application.
Share Care Plan PDF update
Merative Integrated Care
You can now download an updated share care plan PDF that includes a client's notes, touchpoints, and barrier comments to share with the care team, client, or external parties. Note: Your administrator must configure the note, touchpoint, and barrier comment information to display in the PDF. The updated PDF is also available to the share care plan API so that users of external systems can see a client's notes, touchpoints, and barrier comments.
New client summary card
Merative Integrated Care
So that you can see important summary information about a client's current active medications, a new client summary card called Current Medications is now shown on their Summary view. When you capture a client's medication information, the card displays the medication name, strength and frequency. By default, up to five records are shown, but your administrator can configure more records to display.
Assigning and reassigning clients to other care team members
Merative Integrated Care
So that you can manage the caseload of other care team members if needed, you will be able to assign or reassign clients to yourself or to other care team members. To quickly transfer the caseload to another care team member, you will be able to bulk assign or reassign up to 25 clients in a single action.
Autogenerated alerts from assessments and questionnaires
So that you do not need to manually create an alert based on a client's answer to a question or an assessment measure, the alert is automatically triggered (if configured). When you complete the assessment or questionnaire, the alert displays on the client's Summary view or on each care team member's My Alerts page. Note: Your administrator must configure the alert and who will be notified of the alert.
New touchpoint filter options
So that you can identify all clients that have not been contacted recently, the following new filter options are now available:
- No Touchpoint in last 7 Days
- No Touchpoint in last 14 Days
- No Touchpoint in last 30 Days
- No Touchpoint for over 30 Days
The options are available on the Clients list for a care team member. You can apply them to all clients who are assigned to care team members.
Share Care Plan template update
Merative Integrated Care
So that care teams can assess a client's notes and touchpoints in a downloaded share care plan PDF, you can now configure section attribute criteria for notes and touchpoints in share care plan templates. This ensures that only the most relevant information is shown in the Notes and Touchpoints sections of the PDF. You can also include comments as section attribute criteria for barriers.
Configuring the Current Medications client summary card
Merative Integrated Care
So that care teams can see a summary of a client's current active medications, a new client summary card called Current Medications is now available. By default, the card is shown. You can display up to 10 records on the card, show or hide it from view and reorder the card.
Configuring autogenerated alerts from assessment and questionnaires
Merative Integrated Care
So that care teams do not need to manually create alerts, you can configure alerts to automatically generate when you complete an assessment or questionnaire. You can select the trigger for the alert to be created, such as the answer to a specific question or assessment measure and also who is notified of the alert.
New alerts data
Reporting
So that you can report on a client's alerts, new data sets called Alerts, Alert Comments, and Alert Notifications are now added to the Individuals folder. You can use these with the Individuals Demographics data set to display a client's alert data, such as the alert name, priority, status, comments, and who is notified of the alert.
New programs data
So that you can report on clients with and without assigned programs, a new data set called called Programs is now added to the Individuals folder. If you want to see only clients in programs or only clients who are not in programs, you can apply filters to the program data in your report.
New email address data
Reporting
So that you can include a care team member's email address in reports, a new email data item is now added to all Care Team, Users, and Primary Care Team Member data sets in the Reporting folders.
New medication data

To match the medications data that care teams can record in Merative Integrated Care, the Medications data set is now updated. The following new data items are now available:
- Medication Ordered By
- Medication Ordered Date
- Medication Prescription Number
- Medication Prescribed Date
- Medication Prescribed
- Medication Fill Date
- Pharmacy
- Medication Review Status
- Medication Last Refill Date
- Medication Refills
- Medication Refused Reason
New client status history data
Reporting
So that you can report on a client's status history, a new Client Status History data set is now included the Individuals and Individuals Not in Programs folders. You can use this data set with the Individuals or the Individuals Not in Programs data sets to report on a client's status changes, such as when and why the client was deactivated in Merative Integrated Care.
Merative Integrated Care
- When client data is received through an API, no value displays in the Updated By field. The Updated By field should display External System User when the data is received from an external system. (INC3186804, 266755)
Reporting
- When you create Individuals reports that include the Last Successful Login data item from the Care Team data set, no data displays. (264941)
- If you create a report that contains height data, measurement time data is not displayed. (250238)
Merative Integrated Care
- When you filter Notes Highlights for the last month, highlights from over a month ago are displayed in addition to highlights for the last month. (245154)
- If you try to complete certain actions, for example, create a referral or assign a program, and you do not have access to the client, you cannot complete the action and no error message is displayed. If you are sure that you need access to the client, either request emergency access to the client or ask a care team member to add you to the client's care team. (259832)
- When selecting to show or hide a source or original source system in the Administration workspace, in some instances, an incorrect error message with invalid codes is displayed. Ignore this message and continue to show or hide the selected value. (261380)
- If you edit a date data attribute for a client data type, and select Current Date Comparison, then Custom Message, and enter an incorrect message ID, the custom message fields are no longer visible. To see the fields, select None, then re-select Current Date Comparison and Custom Message. (264393)
- When you configure a client data attribute to not use the default value, the value is still visible to care team members. As a workaround, care team members can delete the default value and enter another value. (263255)
- When you configure an information message for a client data type cluster, the message is displayed twice. To avoid this, you can add a description to the data cluster instead of an information message. The description displays to care team members below the cluster title. (263229)
- If you delete a data attribute from a client data type and that attribute was used in a duplicate validation, the validation is not automatically deleted. If you encounter this issue, you can manually delete the validation. (264986).
Reporting
- Assessment and questionnaire data is directly linked to each client.
- When you create a report that contains assessment or questionnaire data, and program data, the assessment or questionnaire data is displayed for all programs to which the client is assigned. This behavior occurs regardless of whether the program contains that particular assessment or questionnaire content. (261299)
- When you create assessment and questionnaires reports and include Assessments by Program data items or Questionnaires by Program Name data items, no data is displayed. (INC2413936, 261278)
- When SSO is enabled, if you sign out of the reporting application, you are automatically signed back in. If you experience this issue, sign out of the Merative Integrated Care application, then sign out of the reporting application. (263876)
Connect 360
- When you search for a client in both systems a misleading error message is intermittently displayed. The message indicates that the search results show only Care clients. The message displays only when the client you searched for was originally registered in and subsequently registered in Connect 360 automatically. Ignore this message, the client exists in both systems and is shown in the search results as expected. (256209, 252716)
